Hawk Fish Emergency

I have a long nose hawk that Ive had about 8 months, he was one of our first residents. I noticed he wasn't really eating or very active last week so I moved him into an emergency tank, but he still isn't eating or moving. He is starting to sort of breath shallow, I'm pretty worried about him at this point. Everyone else in the tank is doing great, its just him. I haven't lost any fish and run the following parameters. Working on raising nitrates and phosphates. Has anyone else had this type issue with a hawk fish before, any help would be greatly appreciated. Thanks

A freshwater dip to check for flukes is an easy first step. Even if it's something else it may give him temporary relief.
