hole in plate coral

Anybody ever experience a hole in the skin of the plate coral. Mine has not fully opened for about a week now. I noticed a small hole in about a 1" from his mouth. He is now stopped taking krill or mysis shrimp. His mouth is open to were you can see the skeleton. I have noticed since this that my nitrate in climbing. Don't know if he is causing the nitrate or not, just seems to me he is on the way out. Is it time to pull him or do we give a chance?
 
thanks for the reply. I have moved him to the back corner, not very shaded but located in the least amount of light. I do not want to pull him. He is about 7" across when he is fully extended. My biggest concern was the nitrate increase. Where he was located, the surrounding duncan and frogspawn were closing up only on the sides closet to the plate. I think I remember someone saying they are toxic but only to the touch. Did not know is the hole may allow the toxins to release in the water or if that is even possible.
