After a 2 minute distilled dip, 6 polyps are opening and some small quantity of organic/living matter dropped off in the water. I can't identify it without a big magnifier.
 
It's almost dead, I think. I don't know what to do to revive it before it goes for good. I screwed up and couldn't keep it happy. Sad.
 
How much light did you put it under? Ricordias, especially Yumas tend to do better under a little shade or in lower lit tanks.

I put it under a "mini cave". No luck though. I don't have a separate tank or fuge, so I'm gonna pull it for burial.
