I'm setting up a hospital tank and want to know should I put sand or substrate in the bottom. I have gotten mixed reviews from the internet.

IMO bare bottom. You want to be able to see anything that may be coming off the fish. Plus, if you medicate it will absorb into the sand and may affect later inhabitants.

Bare bottom w PVC. Aside from what was mentioned above, you can monitor how much the fish eats, remove waste easier and monitor digestion as well...
 
It depends! Are you wanting to set up a hospital tank because you have a sick fish and want to treat it? If so then keep it bare and simple, just flow and heat. Use a HOB filter for flow. If you really mean a QT, then that's different and still depends on the type and size of fish you want to hold in it. What are you wanting to accomplish?
