looks more like a cut than a tear.

If you didn't cut it, take a close look for nudibranchs. They eat the tissue and look just like the corals they eat.

Looks like this now.

I haven't added anything new to the tank. There are snails and a peppermint shrimp I the tank. BUT I just caught the peppermint nibbling on the hammer. I have the worst luck with those jokers..
