Mini Crash. Help needed.

I have a Hannah Checker Phosphate Colorimeter you could borrow if you like. I have never had it tested for accuracy but reads 0 for my tests. I run BRS carbon and gfo mixed in a single reactor. Stirring up crud in your sump might have polluted your water column. The poly filter seems interesting, although I have no experience. You could also have some of my premixed carbon/gfo if you want to give it a shot. Seachem Sea Gel is a similar product I've used that works with a single reactor.
I'm also using rscp from the group buy and have to dose to keep my mg, kh and ca up. Changing salts seems a little dramatic without confirming it is your problem. I'd recommend testing some freshly mixed water. Good idea on mixing the salt but if I were you I'd mix and test the next batch before mixing the salt. That way if that is your problem you'll know. You could then dump that batch, mix the salt, make another batch and retest.
I'm not able to head up that way today due to my wife having to register my daughter for kindergarten when I get off work but you could swing by here or I could bring it up for you later in the week or this weekend.

Thanks Kenny. I appreciate the offer. I am going to stop and have Jenn run a complete test on the way home tonight and see it everything is in line with my tests. That may give me a good start in finding a problem. I will also mix a small batch tonight and see what I come up with.

I know many have great success with Red Sea Coral Pro salt, but it just did not seem to agree with my tank.
A month or so after I starting using it, I had a big problem - took what seemed like forever to clear my tank up and I lost a few corals in the process.
I have switched back to IO/RC and my tank is doing much better. I am not certain exactly what the problem was, I think that it had too much mag for me.
Anyway, I hope you figure it out - it is a hard process

Sorry to hear that you are having difficulties Rich but you'll pull through. If I can help you in any way let me know. I really don't know anything but if there is anything that I can do to help, I will gladly do it. Hang in there 
Thanks for all the good thoughts and offers of help. Means a lot. I have a few things in mind that may be causing some of the issues if the test results pan out like I think they will. I will keep you all posted. Don't want to lay suspicion until I am pretty certain of it.

Me too. One of the worst losses is a pink birds nest. It was soft ball size when I got it in June. Now it is 4" wide by 6" tall. I have given away a few frags of it. The other is a gorgeous torch that was bigger than a soft ball.

Rich this might be a long shot but are you running RODI water and if so how old is the DI resin? It can be possibly leaching bad stuff back into the water that will not pick up on any test kits. Another member on here had the DI do that and he started loosing coral of all different types but some lived through it and also algae started forming in the tank (All test read normal range from multiple different kits). It was also reading 0 tds on the meter so it might be worth a shot and sorry to see anyone having tank troubles, but after reading and looking at params it was the only thing that comes to mind!
