I had one explode a long time ago, there was very loud popping sounds that I heard from the next room over, smoke coming off the water, and a nasty burnt seawater smell.
I realized it before I had finished dumping the first gallon of water into it. I quickly unplugged it and took the electrical piece out of the water. Then I had to fish the remaining pieces of glass out.
This may sound crazy but my heater did not fit in my sump very well so I put it in the overflow. I have not regretted it, I do plan to put a smaller one in the sump as a back up though.

That's actually not that crazy. I had a HoB filter ( tetra I think) that had a heater specifically made to fit in the tube that sucked water into filter. I never had any issues with water temperature.
