Acanthophyllia has something under it.

I say put a camera up and wait to see what bites at it. If its one after another, chances are its a fish switching between coral. Pests dont kill coral in a day, let alone jump from species to species.

The worms you found is because it was a dying coral and they were attracted to the smell.

If the fox face murdered your leather, then its very likely its killing the other coral too. The only reason you dont see it, is because whem the fish see you by the tank, they are more worried about you feeding them...try staying somewhere in the room where they cant see you. Guarantee you will find the culprit in 15 minutes.
 
Now this one looks unhappy. I've moved things to what should be lower flow areas. For some reason it seems to be getting a lot more flow at the bottom left of my tank despite not having change a thing.

I didn't take pics but I'm seeing a lot of these vermatid snail on rocks and on the backs of some snails. I'm getting some bumblebee snails tomorrow and going to try and move as many of my other snails to the sump for a little while.

The biggest problem is what to do with my maxima clam in the mean time. I don't want to get it killed and I certainly don't want to lose anymore acanthophyllia. They're not cheap.1000005756.jpg
 
Vermatid snails irritate, but not to the point where they would be killing off multiple LPS coral. There has to be something else going on. Do you ever take a flashlight to it in the middle of the night? It’s how I’ve found and managed quite a few different things, both bad and good.

Yeah, I do it frequently. But more so in the am since my lights don't come on until noon. I've seen a few worms in the sand but that's about it. When my foxface was eating my toadstool it was during the day like he was going to the kitchen for a snack.

I do it with a blacklight. That doesn't seem to scare things off like a regular light... except the foxface, he turns all kinds of crazy colors/patterns and his spines flare out. 😅
